Requirements for New Students
Fall Semester
- Deadline for submitting health forms: July 1
- Required Student Health Insurance waiver/enrollment period: May 15—August 1st
Spring Semester
- Deadline for submitting health forms: October 1
- Required Student Health Insurance waiver enrollment period: November 15—January 1st
Dear Incoming Oxford Student:
Welcome to Oxford! We hope that your years here will be healthy, happy and always intellectually stimulating. Required health forms are now ready! Please make sure to login to your Emory University Student Health Portal to complete these requirements.
Required immunizations:
- 2 doses of MMR (or 2 doses of individual vaccines)
- Tdap or Td (within last 10 years)
- 3 doses of Hepatitis B
- 2 doses of Varicella
- 1 dose of Meningococcal ACWY (one dose on or after 16th birthday)
Although not an immunization, students are required to complete Tuberculosis (TB) screening as listed on the immunization form.
*All vaccines and doses must be given according to the CDC dosing schedule guidelines.
All vaccinations and any needed lab work are to be completed prior to matriculation. Exception: If necessary, multi-dose vaccination series started prior to arrival at Oxford can be completed, and a Tdap booster given at Student Health Services.
Online Health Insurance Requirement/Waiver
Students are required to have health insurance. All students are enrolled in the Aetna Emory Student Health Insurance Plan every year. Students have the option to waive out of the plan by providing documentation that their personal plan meets the minimum criteria set by Emory. Students must successfully complete the waiver process online via your account prior to the first day of classes. The waiver process must be completed online via OPUS each year. Required Forms
All required health forms can be found within the Emory Student Health Portal. For students over the age of 18, you are able to sign and submit these forms online without downloading the forms.
For students under the age of 18, a parent or guardian is required to sign the form with you. These forms can be downloaded from the list below, or from your Student Health Portal. Instructions to submit these forms can be found there as well.
Turning in Required Forms
Once again: We must receive these forms by July 1st for Fall start students and October 1st for Spring start students.
Once all of your requirements are met, Student Health will be removed from your OPUS To Do list. Check your Emory.edu email for updates on information still needed. If it is still listed, your record is being reviewed or has incomplete or missing items. Check your Emory.edu email for updates on information still needed. A registration hold will be placed on students who have not cleared their Student Health “To Do” list item.
Staffing for processing Entrance Medical Records is intermittent throughout the summer. All questions and correspondence regarding forms will be handled through the patient portal. Once all documents are received and approved, your OPUS account will updated and the Student Health item will be removed. This may take up to two weeks. We greatly appreciate your patience.

We do take walk-in patients when open slots are available. We make every effort to accommodate sick students with same day appointments. If you become ill during the night or early morning, please call as soon as possible for an appointment.
